Your weight is coming down. Your appetite has almost disappeared. Yet climbing stairs feels harder, and lunch is often just a few bites. If this sounds familiar, a lower number on the scale may not tell the whole story.

Mounjaro contains tirzepatide, a medicine that acts on two appetite-related signals, GIP and GLP-1. It can help treat obesity and type 2 diabetes when appropriately prescribed. This guide focuses on eating well during treatment, including for people using other GLP-1 medicines. It is not a guide to choosing or dosing injections.

Get medical help first: Severe or persistent stomach pain, repeated vomiting, inability to keep fluids down, fainting or confusion need prompt medical assessment. Severe symptoms warrant urgent care. If you use insulin or certain diabetes tablets, low blood sugar can also cause weakness. Follow your prescribed low-sugar action plan and contact your treating clinician. Low appetite is a reason to adjust your food plan

A large meal may feel impossible now. Simply cutting your old meals in half can also leave less room for foods you need. The practical goal is to make smaller portions more useful, then check how your body responds.

Does losing weight mean you are losing muscle?

Some lean tissue loss can occur during weight loss. But lean mass is not a direct measure of muscle alone. It includes other non-fat tissues and water. A single body-composition reading cannot explain why you feel weak.

In the SURMOUNT-1 body-composition substudy, about 75% of weight lost with tirzepatide was fat and 25% was lean mass. These were group averages, not a prediction for you. Participants also reported improved physical function despite losing some lean mass.

Take note if daily tasks become harder. Tell your clinician about changes in stair climbing, carrying shopping or getting up from a chair. Review these changes alongside your weight trend, food intake and medical history.

Indian foods that make small meals count

You do not need a separate โ€œinjection dietโ€ made from imported products. Start with foods you enjoy and tolerate. The ICMR-NIN Dietary Guidelines for Indians support variety across grains, pulses, vegetables, fruit, milk or curd, nuts and seeds.

  • Vegetarian options: Thick dal, soft-cooked moong, tofu, paneer or plain curd. Try the texture that feels easiest. Large bowls of pulses may feel uncomfortable when you are bloated.
  • Eggs or non-vegetarian foods: Soft-cooked eggs, lightly cooked fish or tender chicken can be options. Choose less oily preparations if rich food worsens nausea.
  • Keep some carbohydrate: Rice, roti, oats or other grains can fit. Portion size should reflect your appetite, activity and blood sugar needs.
  • Add produce you tolerate: Cooked lauki, carrots or beans may feel easier than a large raw salad when your stomach is unsettled.

For example, add plain curd to a small portion of moong khichdi, or pair a phulka with tofu and cooked sabzi. These are food combinations to adapt, not a complete diet chart.

When appetite is normal, a useful visual starting point is about half a plate of vegetables, one quarter protein foods and one quarter grains. When intake is very low, do not force this ratio. Eating enough takes priority. You also need not fill up on salad before reaching the protein part of your meal.

Try a few bites of your protein food early in the meal if you fill up quickly. There is no compulsory eating sequence. Your dietitian can adjust the balance without making meals a daily struggle.

What helps when nausea or constipation changes your meals?

The 2026 dietary-management review supports smaller portions, slower eating, less fat per meal and adjusting fibre to symptoms. Sip fluids regularly. Follow any fluid limit prescribed for heart or kidney disease.

During nausea, try a less oily preparation before giving up on a food altogether. Swap a rich paneer curry for a simpler tofu or paneer dish. Increase fibre gradually as tolerated; a sudden pile of bran, seeds or raw salad may make discomfort worse.

If symptoms stop you eating or drinking, contact your prescriber. A nutrition consultation can help with food choices, but it cannot replace assessment of a medication side effect.

Plan for strength and maintenance, not just weight loss

The 2026 EASOโ€“EFADโ€“ECPO consensus puts nutrition care, physical function and regular monitoring alongside obesity medicines. A useful plan asks: Can you eat enough? Are your meals varied? Are everyday tasks becoming easier?

Strength activity should suit your health and starting ability. A clinician or qualified exercise professional can help you choose safe movements. If you feel faint or unwell, seek assessment before pushing through exercise.

Also discuss long-term treatment with your prescriber. In SURMOUNT-4, adults without diabetes switched from tirzepatide to placebo regained an average 14% of their week-36 body weight over the next 52 weeks. Continued treatment produced further weight loss. This finding does not mean diet advice can guarantee prevention of regain.

Frequently asked questions

Does Mounjaro cause muscle loss in everyone?

Weight loss can include some lean tissue loss, but lean tissue is not the same as muscle. Changes vary between people. Food intake, strength and physical function should be assessed alongside weight; weakness alone does not prove muscle loss.

Can I eat rice and roti while taking Mounjaro?

Yes. Rice and roti can fit into an individual meal plan. Combine tolerable portions with a protein food and vegetables. When appetite is very low, the priority is meeting your nutrition needs, rather than removing all carbohydrates.

Do I need protein powder on Mounjaro?

Not automatically. First review what you can eat, your kidney health and your daily needs. A dietitian may suggest a supplement if food is not enough, but it should not replace a varied diet without a clear reason.

Should I stop Mounjaro if I feel weak?

Do not make medication changes on your own. Contact your prescriber to assess weakness, low intake and possible side effects. Severe pain, repeated vomiting, fainting or confusion need urgent medical attention. Nutrition support complements medical care.
